My fee is $200 for up to a 50-minute session. 

I am in network with Aetna, United Health Care, Premera, Blue Cross, Regence Blue Shield, Cigna, First Choice and First Health Insurance, 

and I do offer sliding scale fees when needed. 

If you pay upfront instead of using insurance I do offer a discount.

Since payment for service is ultimately the clients' responsibility, please call your insurance company before we meet and gather the following information:

 

• Is Karen David NPI 1659772838 an in-network provider for my insurance plan?

• Do I have insurance coverage for nutrition counseling or medical nutrition therapy, CPT codes 97802, 97803 and 99404?

• Do I have coverage for preventative nutrition counseling, eating disorder treatment, IBS?  If no, which medical conditions are covered?

• Do I need a physician’s referral?

• Do I have a deductible to meet before insurance will pay?

• Do I have a co-pay?

• How many nutrition sessions do I get per year or per lifetime?

• Are telehealth visits covered?

Please note: A valid credit/debit card on file is required and will be used for copays or deductibles as directed by your insurance company and in case of late cancellations or no show. You may also keep an HSA/FSA card on file for eligible transactions.
